Foundation Participation Program
The Foundation believes that advancing medical knowledge and improving individual health can occur together.
Our Participation Program brings together individuals who wish to support the Foundation's mission while contributing to a better understanding of health, healthy aging, and longevity.
Participants may choose to engage in comprehensive health evaluations and, with their permission, allow de-identified health information to contribute to the Foundation's research and educational mission.
A written acknowledgment of your charitable donation will be provided for your tax records.
Comprehensive Donation
$5000.00
- The Comprehensive Level donation comes with:
- Health evaluation history interview of no time limit;
- Review of your existing medical records, medicines and supplements;
- Additional comprehensive medical tests and their evaluation;
- Written personal health optimization plan to include medication, diet and exercise program;
- 24/7 availability via phone or text for any medical questions for 1 year;
- Quarterly review with you of your health plan and progress of unlimited time.
- The Comprehensive plan is renewable annually at $4500.00 donation.
- Personal data collected can be used for research.
Standard Donation
$1750.00
- The Standard Level donation comes with:
- Health evaluation history interview of no time limit;
- Review of your existing medical records, medicines and supplements;
- Additional comprehensive medical tests and their evaluation;
- Written personal health optimization plan to include medication, diet and exercise program;
- Follow-up appointments are a donation of $300 with a 60-minute telehealth appointment.
- Personal data collected can be used for research.
Foundation Supporter

- We ask that you complete our medical questionnaire when making a donation at the Foundation Supporter level.
